Patch to add accelerator key support to output window

This patch adds accelerator key support to output window. Tested on Windows-7
compiling with TDM-GCC 4.8.1, but should apply to all platforms.

An executable version including this patch can be downloaded from
www.xylanta.com.

This follows on from the patch in 1001965.

Additional patch to add accelerator keys to output window menu

The second minor patch adds the accelerator key sequences to the right-click
menu
